National Confidential Enquiry into Patient Outcome and Death (NCEPOD) have been commission by HQIP to conduct a clinical outcome review programme on sepsis, a review of the process of care received by patients with known or suspected sepsis . The aim of this study is to identify and explore avoidable and remediable factors in the process of care for patients with known or suspected sepsis.

There has been a 29% rise in suicide since 2006 among men under the care of mental health services in the UK, a report for the Mental health outcome review programme by The National Confidential Inquiry into Suicide and Homicide by People with Mental Illness (NCISH) shows. The number of suicides in male patients has now reached 1,239 per year.

The Sentinel Stroke National Audit Programme (SSNAP) for the first time today, launches an audit report on the organisation of care for stroke patients once they have left hospital. National congenital heart disease audit reveals 30-day surgery survival rates

Data published today by the National Congenital Heart Disease Audit (NCHDA) shows that 22,719 congenital heart operations and transcatheter interventional procedures were performed across 34 centres, including all 14 specialist paediatric units, between 2011 and 2014 in the UK and Republic of Ireland. No hospital had excess mortality for paediatric surgery within the reporting period as assessed by using the PRAiS risk adjustment model.
