The aim of this template is for users to populate the document with information specific to their Trust or organisation, using the suggested headings and examples as a guide.
The report template consists of:

- ‘At a glance' section headings
- A template for reporting on clinical audit and effectiveness projects.

Guide for completing the Annual Report Template
This document aims to give users some guidance on how to use the template annual clinical audit report. The headings contained within the template are discussed in more detail in the guidance document to give users an understanding of the type of information that can be included.
These documents are to be used as a guide and can be adapted for local use. Whilst we have endeavoured to put together a template which is transferable across different NHS settings, there will be some sections which users may choose to omit from their report if they do not feel they are relevant to their particular organisation.

If you choose to print single-sided, blank pages will appear within the document. Furthermore, HQIP tools and templates are designed using Microsoft Office packages and may lose formatting should they be opened in other formats.
